Last two (2) years W-2 tax form
Last two (2) years 1040 tax returns (if self employed,
own rental property or receive a pension)

Liabilities
Latest 1098 mortgage statements, payment books on any loans.

Assets
Last month bank statements (checking/savings)
Last month investment and/or retirement statements.
